How to Apply at WHALE Respite Center
Click the link below to complete the application. There is an option to translate the form to another language in the upper right hand corner.
WHALE office staff will call your family to schedule a tour.
Tour is completed at WHALE and during the tour, the parent/guardian completes the registration and all necessary paperwork.
Family provides WHALE with the child’s birth certificate and most recent immunization record and physical.
If the family will be using CCCAP, there may be a prior authorization process.
